Transjugular intrahepatic portosystemic shunt (TIPS) is a sophisticated, minimally invasive surgical treatment for chronically and often terminally ill patients with portal hypertension and liver cirrhosis.
TIPS surgery has been implemented in a limited number of international clinics due to the complexity of the technique and the need for a highly qualified team of physicians specializing in the treatment of complications of liver cirrhosis and portal hypertension. The procedure
involves creating a bypass (an additional blood flow pathway) through the liver, connecting the portal vein to the hepatic vein, and maintaining the lumen of the implanted shunt open with a stent. Once the bypass is starting to function the pressure in portal vein is decreasing and the symptoms are improving dramatically
Indications for TIPS:

Bleeding from esophageal and gastric varices

Prevention of high-risk recurrent bleeding due to gastric and esophageal varices;

Portal gastropathy (venous blood stagnation in the stomach walls, threatening bleeding);

Hepatorenal syndrome;

Budd-Chiari syndrome;

Refractory ascites (fluid accumulation in the abdominal cavity);

Preventive surgery before liver transplantation if there is a risk of complications from portal hypertension;

Right-sided hydrothorax
Contraindications:
- Heart failure, pulmonary hypertension, severe sepsis, biliary obstruction, multiple liver cysts.
The decision to perform TIPS is being made by a team of physician, surgeons and anesthesiologists, taking into account the individual patient's characteristics.
TIPS surgery is performed under intravenous anesthesia. After the surgery, the patient remains under the surveillance of doctors at the Scandinavian Health Center for several days and can then be discharged home. Full rehabilitation takes up to 2 weeks, depending on the extent of the procedure and individual characteristics.
The cost of treatment can vary from 1 000 000 rubles to 1 500 000.
